All new patients need to fill out the Health Questionnaire, the Confidential Patient Information, and the Terms of Acceptance. Please print and fill out all forms before the day of your appointment.
Once they are complete you can fax them to (425)-452-9306, if you are not able to fax them you may bring them with you at the time of your appointment. Feel free to call the office if you have any questions!
